UPDATES ON THIS SERVICE

  • Due to physical distancing measures being implemented, civil registry documents / applications may either be submitted in person (by APPOINTMENT only) or by POST (each copy of the application form must be notarised by a public notary).  
  • Applicants who wish to submit their requirements in person must also bring with them a printed copy of their confirmed appointment during their visit to the Philippine Embassy.
  • For health and safety reasons, we only accept payments via card or contacless.  Cash payments will not be accepted at the moment.
  • Only those with confirmed appointments will be allowed to enter the Embassy premises to avoid overcrowding.
  • Collection of documents will only be done by post in strict compliance with physical distancing measures being implemented.  All applicants must submit a self-addressed, stamped, special delivery envelope with tracking number together with their documentary requirements.
  • Incomplete submissions will immediately be returned to the applicant or destroyed through a secure process in compliance with relevant data protection laws

The death of a Filipino citizen in the United Kingdom or Ireland must be registered with the Philippine Embassy. The Report of Death is equivalent to a Philippine death certificate.  It will be sent by the Philippine Embassy to the Philippine Statistics Authority (PSA), the Philippines’ main civil registry authority.

A Report of Death may be filed in person or by post / mail.  If filed by post/mail,  all copies of application forms sent by post must be notarised by a public notary. Informants who wish to file their civil registry reports by post / mail may opt not to include the original supporting documents in their submission.

Requirements:

  1. A confirmed appointment of the individual submitting the report (Book here). [This requirement only applied to those who will be filing the report in person]
  2. Five (5) copies of the accomplished Report of Death form
  3. Original and five (5) copies of the British / Irish Death Certificate
  4. Five (5) copies of the data page of the deceased individual’s Philippine passport
  5. Processing fee of GBP 23.00
  6. Applicants from Ireland who are sending their documentary requirements by post must pay the relevant processing fees through a bank draft payable to the Philippine Embassy in London.
    • Applications filed by post / mail must be paid via POSTAL MONEY ORDER (UK residents) or BANK DRAFT (Ireland residents).
    • Payee must appear as PHILIPPINE EMBASSY LONDON

Steps (for on-site submissions):

  1. Book an appointment here.
  2. Prepare all the necessary requirements and supporting documents.
  3. Visit the Philippine Embassy and proceed with the following steps:
    • Submit complete sets of your requirements and supporting documents to the processor.
    • Pay the processing fee at the cashier (we only accept payments by cash or contactless)

Important Reminder:

  • If the report is filed by post or mail, do not send us the original supporting documents.
