PASSPORT APPLICATION
Step 1: Accomplish the application form
Step 2: Prepare the following supporting documents (printed):
New passport (adult): 1. PSA birth certificate, original and 1 photocopy 2. At least 1 valid ID, original and 1 photocopy *If the applicant has a machine-readable passport (maroon, no chip), machine-readable ready passport (green), or any older passport, his/her application shall be considered new application.

Renewal of epassport: 1. Original passport 2. Photocopy of passport data page *If the applicant’s expired passport was not renewed in 2 years or more, submit an Affidavit of Late Renewal (£23.00).
*If reacquired/retained Filipino citizenship (dual), include photocopies of the Identification Certificate, Order of Approval, and Oath of Allegiance. These are the documents issued during the oath-taking ceremony. |
Lost passport: 1. Photocopy of the passport data page, 2 copies 2. PSA birth certificate, original and 2 photocopies 3. PSA marriage certificate (for married women only; please see below), original and 2 photocopies 4. 2 valid IDs, original and 2 photocopies each 5. Affidavit of Loss (in English), original and 2 photocopies – if to be notarized at the Embassy, fee is £23.00 6. Police Report *If reacquired/retained Filipino citizenship (dual), include photocopies of the Identification Certificate, Order of Approval, and Oath of Allegiance. These are the documents issued during the oath-taking ceremony. |
Minor applicants: 1. Original passport (if renewal) 2. Photocopy of passport data page (if renewal) 3. PSA birth certificate, original and 1 photocopy 4. If born abroad and report of birth was registered less than 1 year ago, the report of birth, original and 1 photocopy 5. Parent’s passport, original and 1 photocopy of data page, or if the minor is illegitimate, the mother’s passport, original and photocopy 6. If parent is unable to appear, an Affidavit of Consent to Travel and Special Power of Attorney authorizing the representative to assist in the minor’s passport application; photocopy of the parent’s passport data page; and the representative’s passport, original and 1 photocopy of data page 7. Additional identification documents of school-age minor (e.g., school ID), original and photocopy 8. Parent's Marriage Certificate (if accompanied by Father) *If reacquired/retained Filipino citizenship (Derivative), include photocopies of the Identification Certificate, Order of Approval, and Oath of Allegiance. |
Damaged/Mutilated passport: 1. Photocopy of the passport data page 2. PSA birth certificate, original and 1 photocopy 3. PSA marriage certificate (for married women only; please see below), original and 1 photocopy 4. 2 valid IDs, original and 2 photocopies each 5. Affidavit of Explanation (in English), original and 1 copy– if to be notarized at the Embassy, fee is £23.00

Dual Citizen’s new passport: 1. Original recent Philippine passport 2. Photocopy of passport data page 3. PSA birth certificate, original and 1 photocopy 4. PSA marriage certificate, original and 1 photocopy (if applicable) 5. Identification Certificate, original and 1 photocopy 6. Oath of Allegiance, original and 1 photocopy (for principals only, not dependants) 7. Order of Approval, original and 1 photocopy (for principals only, not dependants) 8. At least 1 valid ID, original and 1 photocopy |
Married women who will use husband’s surname or hyphenated name: 1. Original passport (if renewal) 2. Photocopy of passport data page (if renewal) 3. PSA birth certificate (if new application), original and 1 photocopy 4. PSA marriage certificate, original and 1 photocopy |
Women who are widowed, annulled, divorced abroad, or whose marriages were declared void, and will revert to maiden name: 1. Original passport 2. Photocopy of passport data page 3. PSA marriage certificate with annotation of annulment, divorce, or declaration of nullity decree (if applicable), original and 1 photocopy 4. Death certificate of husband (if applicable) – PSA death certificate if husband is Filipino, original and 1 photocopy |
Step 3: Walk in to submit application and complete supporting documents, Mondays to Fridays, except holidays.
Step 4: Pay the passport fee at the cashier, by card or contactless:
- New/Renewal: £55.00
- Lost/damaged/mutilated: £137.00 (ePassport – with electronic chip); £82.00 (machine readable – without electronic chip)
Step 6: Go to the data capturing area. Embassy personnel will capture the applicant’s biometrics (i.e., photo, fingerprints, and signature). Earrings, necklace, nose ring, and/or coloured contact lenses are not allowed during the photo capture. Do not wear sleeveless, spaghetti strap, see-through, tube, halter, plunging neckline or similar attire. Male applicants with long hair must tie their hair during photo capture.

- The Embassy has no photocopying services. Applicants must bring complete documents and sufficient copies.
- The processor reserves the right to request additional supporting documents, if necessary and justified.
- Philippine Statistics Authority (PSA)-issued documents bear the seal of the PSA (click here for sample)
- Wear appropriate clothing for the photo capture. Do not wear sleeveless, spaghetti strap, see-through, tube, halter, plunging neckline or similar attire.
- Earrings, necklace, nose ring, and/or coloured contact lenses are not allowed during the photo capture.
