A. Whole Human Remains
Checklist of Requirements
An original copy and one (1) photocopy must be submitted for all documentary requirements listed below:
- Death Certificate issued by the Register Office or, in its absence, a Coroner's Certificate of the Fact of Death
- Embalming Certificate issued by the funeral parlour
- Certificate to the effect that the human remains have been properly placed in a hermetically sealed container or casket (Funeral Director's Declaration stating that the remains have been properly prepared and packed for shipment)
- Certificate that there is no existence of the infectious disease subject to international health regulations (IHR), emerging or re-emerging disease, of public health emergencies of international concern (PHEIC) from the health authority of the country of origin;
- Out of England Removal Certificate issued by the Coroner or its equivalent
- The Declaration must also state the flight details and the consignee's address
- “Certificate of No Objection” from the Philippine Embassy
- Passport of the deceased
- Processing fee of £23.00 upon issuance by the Philippine Embassy of a Consular Mortuary Certificate
B. Cremated Remains
Checklist of Requirements
An original copy and one (1) photocopy must be submitted for all documentary requirements listed below:
- Death Certificate issued by the Register Office or, in its absence, a Coroner's Certificate of the Fact of Death
- Cremation Certificate
- Certificate to the effect that the human remains have been properly placed in a hermetically sealed container or urn (Funeral Director's Declaration stating that the remains have been properly prepared and packed for shipment)
- Certificate that there is no existence of the infectious disease subject to international health regulations (IHR), emerging or re-emerging disease, of public health emergencies of international concern (PHEIC) from the health authority of the country of origin;
- Certificate or declaration from the crematorium or funeral director stating that the urn contains only the ashes of the deceased
- Passport of the deceased
- Passport of the authorized person who will be carrying the cremated remains to the Philippines
- Flight itinerary
- Full repatriation details and consignee’s address
- Processing fee of £23.00 upon issuance by the Philippine Embassy of a Consular Mortuary Certificate
Important Note:
- A Report of Death must be filed if the deceased is a Filipino Citizen or a dual citizen (bearer of a foreign citizenship and a Filipino citizenship).
- For deaths related to COVID-19, the Bureau of Quarantine reserves the right to allow or disallow the transport of human remains whose death occurred during epidemics / outbreaks or the if the cause of death is due to highly pathogenic organisms, infectious disease subject to IHR, emerging or re-emerging disease, and PHEIC (Philippine Department of Health Memorandum No. 2020-67).
- If transport is allowed for the human remains concerned, the following measures shall be undertaken as the case may be to ensure public health safety:
- The remains shall be cremated before shipment; or
- For remains that do not require cremation, the casket shall be permanently and hermitically sealed from the port of origin until burial. The burial must take place within twenty-four (24) hours after the issuance of clearance of admission into the country.
