The Philippine Embassy issues travel documents in cases of emergency.
Requirements:
- 2 copies, New Passport Application Form (Form No. 1)
- Original or photocopy of old Philippine passport
- In the event of a lost passport, the applicant must submit the following documents:
- 2 copies, Affidavit of Loss (the original signature of the Affiant must appear on both copies);
- Original or copy of the PSA-issued Birth Certificate or Marriage Certificate; and
- At least one (1) copy of a document verifying the applicant's identity such as a driver's license, Professional Regulatory Commission Card, etc.
- In the event of a lost passport, the applicant must submit the following documents:
- 4 pieces, passport size photograph
- Processing fees:
- Travel document, £ 28.00
- Affidavit, £ 23.00
- Express fee (optional), £ 10.00
Steps:
- Prepare all documentary requirements before visiting the Philippine Embassy.
- At the Embassy, proceed directly to the Assistance-to-Nationals (ATN) Section and present all documentary requirements to the ATN Officer for evaluation.
- Proceed to the cashier and pay the relevant processing fees.
- Collect the Travel Document in three (3) working days or, in the case of express processing, collect the Travel Document at the Embassy after one (1) working day between 4:00 p.m. to 5:00 p.m.
Important Reminders:
- Personal appearance is required.
- Applicants must surrender the Travel Document issued by the Philippine Embassy together with the Affidavit of Loss upon application of a new Philippine passport.
- The Travel Document is valid for one (1) month from the date of issue and may only be used once.
